Services

1. Upon a request for service, the Nursing Supervisor will visit the client to assess and determine the client's need for services. A Plan of Care is developed to meet the needs of each individual client.

2. The Nursing Supervisor meets with the Home Health Care Coordinator to discuss the clients needs, desires and medical condition. The coordinator then contact the client's physician for a signed Plan of Treatment otherwise known as "A Doctor's Order."

3. The coordinator selects staff to serve the client based on education, experience, cultural interest, and understanding of the medical needs of the client.

4. The selected staff are given orientation on the client's medical, physical, and personal needs. The coordinator confers with the client regarding the selection of staff and a service assignment is established.

5. The Director of patient services performs follow-up communications in person and by telephone to make sure that the client's physical and emotional needs are met. The Nursing Supervisor conducts supervisory visits every six months to evaluate and reinforce the service needs of the client.
